Position Overview
The Clinical Manager is responsible for overseeing the day-to-day clinical operations of home care and hospice programs, ensuring high-quality patient care, and collaborating with leadership to maintain compliance and operational excellence.
Reporting Structure
The Clinical Manager reports directly to the Executive Director.
Key Responsibilities
Conduct annual performance reviews and field supervision assessments for clinical staff.
Oversee staff scheduling, vacation coordination, and coverage.
Facilitate clinical orientation for new field staff in collaboration with HR.
Work with leadership to ensure home care and hospice services align with financial and operational goals.
Coordinate patient transitions between home care and hospice services.
Provide oversight for all patient care services and personnel, including:
Collaborating on patient assignments
Ensuring patient needs are continually assessed
Overseeing care plans and their updates
Manage day-to-day coordination of field staff and ensure effective task management.
Support quality management initiatives under the direction of the Quality Director.
Oversee utilization reviews and ensure compliance with professional practice standards.
Address patient complaints and work to resolve issues efficiently.
Conduct skilled nursing visits and participate in on-call rotations as needed.
Lead interdisciplinary meetings and facilitate training sessions for new and existing staff.
Monitor clinical schedules, documentation, and productivity metrics.
Collaborate on hiring efforts, including job postings, interviews, and onboarding.
Maintain compliance with Medicare Conditions of Participation and regulatory requirements.
Provide ongoing coaching and leadership to staff, fostering a positive and professional work environment.
Qualifications
Associate's degree in nursing required; bachelor's degree preferred.
Active RN license in good standing.
Previous management experience preferred.
Strong understanding of home health and hospice regulations.
Excellent communication, leadership, and problem-solving skills.
Proficiency in electronic medical records (EMR) systems.
Ability to work flexible hours, including weekends if needed.
Reliable transportation with valid driver's license and proof of insurance.
Work Environment & Expectations
Combination of office-based and clinical field work.
Some travel required for patient visits and team meetings.
Remote work eligibility with manager approval.
Must pass background checks and comply with all safety and compliance policies.
